Privacy Policy
This policy explains what information is collected through Trusted Partner Forms and how it is used.
Who operates this service
This website and form platform is operated by Ellis Taylor Marketing, a digital marketing agency based in Baltimore, Maryland ("we," "us," or "the platform"). Each public form belongs to the organization named and branded on that form. When you submit a request, your information goes to that organization so it can review the request and make the introduction you asked for. Ellis Taylor Marketing hosts the platform and processes the information on the form owner's behalf.
If a form belongs to an organization other than Ellis Taylor Marketing, that organization is responsible for how it handles your request after receiving it. Questions about a specific request should go to the organization named on the form; questions about this platform or policy can be sent to the contact address below.
What we collect from form visitors
When you submit a request through a form on this platform, we collect the information you provide: your name, email address, phone number, preferred method of contact, the services you select, per-service notes, and any general notes you add. We also record that you checked the consent box, the consent language shown to you, and the time of your submission.
We do not currently persist public page-view, referral, or service-selection analytics. The form owner's dashboard retains request history submitted through the secured form endpoint.
What we collect from form owners
For account holders who create and manage forms, we collect account email, organization or business name, the form's public web address, recipient email address, form settings, logo or logo text, theme choices, enabled services, custom services, submission counts, and recent submission summaries.
Dashboard access uses secure email sign-in links. We do not ask users to create or reuse a password for this app.
Error and diagnostic logs
If something goes wrong in the app, we keep a technical error log so problems can be found and fixed. An error log may include the error message, error code, page path, browser user agent, and technical metadata such as file name or line number. Public error reports are not written directly to the database, and diagnostic logs do not intentionally include a form submitter's email address. Access is restricted to the site developer.
How we use it
If you are a form submitter, your request information is delivered to the organization that created the form you used, which is the organization named and branded on that form. That organization uses the information to make the requested introduction to its service partners. Ellis Taylor Marketing hosts and processes the data on that organization's behalf.
Once your information has been shared with a service partner by the organization you submitted it to, that partner's use of your information is governed by their own privacy practices.
If you are an account holder, we use your information to operate the service, provide dashboard access, display your form, send submission notifications, show basic reporting, maintain security, and support the platform. Submission emails are sent only to approved recipient email addresses configured for the relevant form.
Service providers
We use trusted service providers for website hosting, database storage, authentication, file storage for public logos, and transactional email delivery. This includes Supabase for database, authentication, and file storage; Resend for notification emails; and our hosting provider for serving the site and API. These providers process information only as needed to provide those services.
What we do not do
We do not sell your personal information. We do not use it for unrelated marketing without your consent. We do not intentionally collect payment details, Social Security numbers, private homeowner records, or sensitive legal, financial, medical, or insurance claim details through this form.
Your choices
Form submitters and account holders can ask to access, correct, or delete information associated with them. To make a request, contact us at privacy@ellistaylormarketing.com.
Cookies and browser storage
The site uses essential browser storage for secure sign-in, dashboard sessions, form settings, local accessibility preferences, and privacy-choice acknowledgment. Form-submitter contact information and request notes are not stored in local browser storage. We do not currently use advertising cookies, retargeting pixels, or third-party marketing trackers. Children's privacy
This site and the forms hosted on it are not directed to children under 13, and we do not knowingly collect personal information from children under 13. Account holders must be at least 18 years old. If you believe a child has submitted personal information through this site, contact us at privacy@ellistaylormarketing.com and we will delete it.
Do Not Track
Some browsers offer a "Do Not Track" (DNT) signal. Because there is no common standard for interpreting DNT signals, this site does not respond to them. We do not track visitors across third-party websites, and we do not use advertising or cross-site tracking technologies.
Security
We use reasonable technical and organizational safeguards to protect the information collected through this site, including authenticated access to the dashboard, database access controls that restrict each organization to its own data, and reputable infrastructure providers for hosting, database, authentication, and email delivery. No method of transmission or storage is completely secure, but we work to protect your information using industry-standard practices.
Data retention
Request submissions are retained for up to 365 days. Limited activity records, when enabled, are retained for up to 90 days. Submission-attempt security records containing a one-way network-address hash are retained for up to 7 days. Unresolved diagnostic logs are retained for up to 90 days, and resolved logs for up to 30 days after resolution. Dashboard access is limited to authorized users for the relevant organization.
